RTO Forms Commonly Relevant to Motor Claims

Only four forms are included here for now. Each has a simple explanation so a policyholder understands why the form exists before downloading it.

Form 26

Purpose: Application for a duplicate Certificate of Registration (RC) when the original RC is lost, destroyed or requires replacement under the applicable process.

When it may matter in a claim: A duplicate RC may become relevant when the original registration document is unavailable.

Form 28

Purpose: Application and grant of a No Objection Certificate (NOC) for a motor vehicle, generally used where an NOC is required for movement/registration-related purposes.

When it may matter in a claim: It can become relevant in vehicle transfer or registration situations connected with the claim circumstances.

Form 29

Purpose: Notice of transfer of ownership of a motor vehicle. It is used to notify the registering authority about a transfer of ownership.

When it may matter in a claim: It can be relevant when ownership of a vehicle changes, including circumstances where an insurer's settlement involves transfer-related documentation.

Form 30

Purpose: Application for intimation and transfer of ownership of a motor vehicle. It is used in the ownership-transfer process with the registering authority.

When it may matter in a claim: It can become important where ownership is being transferred after a settlement or sale/transfer of the vehicle.

Why ClaimVoice explains the form before offering it

Many policyholders are told to “submit Form 29” or “submit Form 30” without being told what the document actually does. ClaimVoice should explain the purpose first, then provide the download.

Important: Forms can be revised. Before publishing the final PDFs, ClaimVoice should verify that the uploaded copies are the latest prescribed versions and that the issuing authority accepts them for the intended transaction.
